This study describe the management of case of proximal splenorenal shunt operated in Hospital Center Soavinandriana. 14 cases operated are included in the study between January 1997 and December 2001. The proximal splenorenal shunt is surgical treatement of portal hypertension. As a whole patient operated comes From rural environment. They are between 30 and 45 old year. The male sex predominate. The association of clinical, biological, and echographic signs suspect the bilharzian origin of the portal
