Beta thalassaemia disease is not common in Madagascar Island. We report the case of a 7-year-old male, who presented with beta thalassaemia major who underwent splenectomy because of enormous spleen volume and increased need for transfusion. The disease status is commented. Indications for splenectomy are discussed. Alternative therapies are reminded. Perioperative conditions are analysed. We conclude then, that splenectomy is performed unsafely in beta thalassaemic infant in Madagascar.
